I don't think it's a surprise future twist. I think the exchange is to be taken at face value. Sam doesn't feel like the shield belongs to him because he associates it with Steve. Period. Steve thinks Sam deserves it because its should be wielded by someone whose name begins with 'S'. Or, that he thinks Sam is the worthiest. One of the two. Even if an alternate timeline Sam were involved, this timeline's Sam never wielded it so he's still right. BTW, the way they explained all this time travel, Steve wouldn't have been able to come back as Old Steve in Endgame because he would have branched off a new universe just by staying in the past. Maybe it's the one where Wonder Woman and Steve Trevor (ah, are we sure he married Peggy Carter and not Diana Prince? Are we sure he wasn't also Buck(y Barnes/Steve) Rogers in this alternate timeline? It's funny to me that Steve Rogers and Bucky Barnes = 2 halves of Buck Rogers) met in WWII.
